As the parent or guardian of the registered student(s), you confirm that the information provided to Evo Sports during sign-up is accurate and complete.
By enrolling your child, you agree to their participation in all activities organized by Evo Sports at any of its locations and acknowledge that they will be using sports equipment provided by the organization.
You also accept that participation in sports activities carries inherent risks and, therefore, agree not to hold Evo Sports, its coaches, staff, or facility providers liable for any losses, liabilities, damages, injuries, or death that may occur while your child is taking part in any Evo Sports activities, except in cases of proven negligence.
